Client Photo Release Form

Below is a link to our Photo Release Form for clients, volunteers or partners to read and sign, granting permission for Children’s Aid Society to share their photo.

Please print the form, and return to our office. You can return the printed copy in person, by mail or by email. When sending by email, scan the signed form into a scanner to save as a PDF or take a photo of the Form and return by email. Please also include any comments or stories you would be willing for us to share along with your Form and permitted Photo.
